Required Documents

Enrollment in SF PATH is based on the ability to obtain proof of both your citizenship/legal immigration status and your identity, in accordance with federal requirements.

If valid documentation of U.S. citizenship status and identity cannot be obtained either by yourself or on your behalf, you will not be able to enroll into SF PATH. However, you may be eligible for the Department of Public Health’s Healthy San Francisco program, a program that does not require proof of U.S. citizenship or legal immigration status.

During your application appointment, you will be required to provide the following documentation:

  1. Personal identification
  2. Proof of residency
  3. Proof of household income and assets

This documentation is required because it helps the Application Assistor determine if you are eligible for SF PATH, Healthy San Francisco or for a public health insurance program. The application process is considered incomplete unless all required documents are submitted.


1. Personal Identification

Documents must contain a photograph. ONLY ONE of the following documents is needed:

  • State Issued Driver License or ID
  • U.S. Passport (expired passports are acceptable)
  • Certificate of Naturalization (N-550 or N-570)
  • U.S. Mariner Card
  • Certificate of Indian Blood/US Amer. Ind. tribal doc.
  • Permanent Resident Card/Green Card (I-551)


2. Proof of San Francisco Residency

Documents must have the applicant's name. ONLY ONE of the following documents is needed:

  • State Issued Driver License or ID
  • Current Pay Stub
  • Bank Statement
  • Utility Bill
  • SFUSD School Registration
  • SSI Awards Letter
  • DMV registration 
  • Pension check / Statement
  • Social Security Award Letter
  • General Assistance Letter
  • Affidavit of Support - This is a form that someone you live with can sign to vouch for your residency. They will need to provide one of the above items in their name.



3. Proof of Citizenship or Legal Immigration Status

Documents must have the applicant's name. ONLY ONE of the following documents is needed:

  • U.S. Passport issued without limitation (expired ones are acceptable)
  • Certificate of Naturalization (N-550 or N-570)
  • Certificate of U.S. Citizenship (N-560 or N-561)
  • U.S. Birth Certificate
  • Permanent Resident Card/Green Card (I-551) with arrival/departure record appropriately stamped (I-94), permit to re-enter United States (I-327)
  • Certification of Report of Birth (DS-1350)
  • Report of Birth Abroad of a U.S. Citizen (FS-240)
  • U.S. State Department Certification of Birth (FS-545 or DS-1350)
  • U.S. Citizen Identification Card (I-197 or I-179)
  • American Indian Card (I-872)
  • Northern Marianas Card (I-873)
  • Final adoption decree showing a U.S. place of birth



4. Proof of Income and Assets

Assets are not factored into your SF PATH eligibility. However, this documentation is needed to determine your eligibility and fees for Healthy San Francisco, the City and County's health acess program, if you are found ineligible for SF PATH:

  • Current Pay Stub
  • Current Tax Return
  • Signed Statement From Employer
  • Bank Statement
  • Rental Income Receipts or 1040 tax return
  • Bank Book / Annuity Statement
  • Unemployment Benefits Statement
  • Veteran's Statement
  • Workers' Compensation Statement
  • Copy of Award Letter
  • Certification from the issuing institution of any non-retirement stocks, bonds, or certificates of deposit.
  • Account statements for retirement accounts, such as 401(k), 403(b) accounts (value of retirement holdings are not counted toward household assets)
