How Appointments Are Made

Enrollees will call their Medical Home to schedule an appointment. The phone number and address are printed on their SF PATH Enrollee ID Card. If they need help finding their Medical Home, they can call SF PATH Customer Service at (415) 615-4510, Monday through Friday, 8:30am to 5:30pm.

Enrollees are told to bring their Enrollee ID Card to their medical appointment. If they need to reschedule an appointment they have been told to notify their Medical Home to cancel and reschedule at least 48 hours in advance.
