San Francisco Provides Access to Health Care (SF PATH) is the City and County of San Francisco’s Low Income Health Program. The Low Income Health Program was created by the State of California in July 2011 as a temporary program for certain Californians who will be eligible for free or low cost health insurance as a part of the federal health reforms that take effect in 2014.

SF PATH is open for enrollment until 12/31/2013. Starting January 1, 2014, SF PATH and its services will end. This is because the Affordable Care Act is providing SF PATH Enrollees with significantly better choices: eligibility in the Medi-Cal program, California's free comprehensive health insurance program or for those with higher incomes, subsidized private health insurance through Covered CA.

Most SF PATH Enrollees are eligible for Medi-Cal and will be automatically enrolled into the program, making the transition to free health insurance easy. For those SF PATH Enrollees eligible for subsidized private health insurance through Covered CA, you must take action and apply to Covered CA.

Which Program Will I Qualify For in 2014? Your income will determine which program you will qualify for in 2014. SF PATH will help you enroll in the right program beginning in October 2013. Here is a guide to these programs:

 


What is it? Immigration Status Income Limits
Medi-Cal A free Federal and State health insurance program U.S. Citizen and certain legal immigrants Family income at or below 133% of the Federal Poverty Level. ($15,282 for one person or $31,322 for a family with four people)
Covered CA California’s health insurance market place where you can compare and buy health insurance on-line U.S. Citizen and certain legal immigrants Any.  You will qualify for a discount (“tax credit”) on your monthly premiums and co-pays if your family’s income is at or below 400% of the Federal Poverty Level. ($45,960 for one person or $94,200 for a family with 4 persons)

If your Family Income is at or Below 133% FPL, SF PATH will automatically enroll you into free health insurance through Medi-Cal. Your insurance becomes effective January 1, 2014. In October of 2013, Enrollees will be mailed informational letters from SF PATH and the state Department of Health Care Services about their enrollment into Medi-Cal. SF PATH Enrollees who move to Medi-Cal will be able to keep their Department of Public Health doctor and go to the same clinic but will have access to more services in Medi-Cal.

If your Family Income is Above 133% FPL, you must apply for health insurance through Covered CA. Starting in October of 2013, you can find out what health insurance is available to you and your family, how much it will cost, and how much of a discount you can receive through Covered CA. Your health insurance will become effective January 1, 2014.
